Howit works: You pull on the string, which pulls the Rack (linear motion), which rotates the Pinion (rotation). The rotation applies tension to the Torsion spring, while also pushes on the first crossing joint of the scissor mechanism. Pushing that joint outward causes the scissor mechanism to extend, which extends the blade.

It's an instant kill that ignores all enemy health however you can't guard with the hidden blade nor do you have any kind of attack with it, also the timing must be perfect. Itwas revealed in the artbook for origins that the blade was originally used ontop of the wrist, which was used for more combat purposes. The blade was used beneath the wrist for stealth purposes. Ontop of the wrist, it can be stealth, but more for open combat. •. I'm thinking it was safe to use even when Bayek wore it.
